Your enrollment name comes from your employer: ask your benefits administrator to update it. Then update your vsp.com profile under Member Details, My Account.

A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
Your account number, and the name the account is in now.
