How to change your name with Anthem
Married or divorced and updating your accounts? Here's how Anthem takes a name change, from its own help page.
In person or by phone only. Anthem is one of the few that won't change your name online. Bring your documents when you go.
Anthem's steps
Plan through work: send the name change through your employer's benefits team. Other plans: call Member Services at the number on the back of your ID card.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for Anthem and every other company on your list, with your certificate attached. It also keeps track of who's done. The checklist is free.
