How to change your name with HealthEquity (HSA)
Married or divorced and updating your accounts? Here's how HealthEquity takes a name change, from its own help page.
HealthEquity's steps
Log in and complete the Change of Personal Information form online, uploading your marriage license, divorce decree or new Social Security card. Your employer can't do it for you.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for HealthEquity and every other company on your list, with your certificate attached. It can even print and mail it to HealthEquity for you: no printer, no stamps. It also keeps track of who's done. The checklist is free.
