How to change your name with Medicare
Married or divorced and updating your accounts? Here's how Medicare takes a name change, from its own help page.

Medicare's steps
Nothing separate to file: Medicare uses the name Social Security has. Once Social Security updates you, Medicare mails a new card. Keep your plan cards.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for Medicare and every other company on your list, with your certificate attached. It also keeps track of who's done. The checklist is free.
