How to change your name with Kaiser Permanente
Married or divorced and updating your accounts? Here's how Kaiser Permanente takes a name change, from its own help page.
In person or by phone only. Kaiser Permanente is one of the few that won't change your name online. Call 1-888-901-4636 or visit, and bring your documents.
Kaiser Permanente's steps
Call the Patient Support Line to change your legal name. Individual plans can also use the account change form with your legal document.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for Kaiser Permanente and every other company on your list, with your certificate attached. It also keeps track of who's done. The checklist is free.
