How to change your name with Florida Blue
Married or divorced and updating your accounts? Here's how Florida Blue takes a name change, from its own help page.
Florida Blue's steps
Bought directly: the primary account holder signs a Change Application and faxes or mails it with your certificate, decree or new license. Marketplace plans change at HealthCare.gov; work plans through HR.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for Florida Blue and every other company on your list, with your certificate attached. It can even print and mail it to Florida Blue for you: no printer, no stamps. It also keeps track of who's done. The checklist is free.
