How to change your name with Delta Dental
Married or divorced and updating your accounts? Here's how Delta Dental takes a name change, from its own help page.
In person or by phone only. Delta Dental is one of the few that won't change your name online. Bring your documents when you go.
Delta Dental's steps
Plans through work: tell HR. Otherwise contact the Delta Dental company for your state (the member company locator finds it), using the number on your ID card.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
