Sign in to your account and report the change within 30 days (marriage, divorce or a name correction). Medi-Cal members tell their county office within 10 days.

A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
Your account number, and the name the account is in now.
