How to change your name with Blue Cross Blue Shield
Married or divorced and updating your accounts? Here's how Blue Cross Blue Shield takes a name change, from its own help page.
In person or by phone only. Blue Cross Blue Shield is one of the few that won't change your name online. Bring your documents when you go.
Blue Cross Blue Shield's steps
Each state's Blue Cross plan keeps its own records: call the member services number on the back of your ID card. Plans through work (including federal FEP) start with HR.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
