Fill in the Residential Name Change form and mail it with a copy of your photo ID. Afterward, set up autopay again: recurring payments are cancelled by the change.

A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
Your account number, and the name the account is in now.
