Download the Name Change Form from Suncoast's forms page and bring it to a branch, or ask a branch how to send it, with proof such as your marriage certificate.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
