Chase's steps
Send documents in your new name (marriage certificate, or new license plus Social Security card) with a signed W-9. Upload them at chase.com/DocumentUpload or mail them.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for Chase and every other company on your list, with your certificate attached. It can even print and mail it to Chase for you: no printer, no stamps. It also keeps track of who's done. The checklist is free.
