How to change your name with New York Life
Married or divorced and updating your accounts? Here's how New York Life takes a name change, from its own help page.
New York Life's steps
Use Self-Service in your online account, or send the Name Change Request Form signed with your new name. Life and annuities: Cleveland Service Center, or fax 800-278-4117.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for New York Life and every other company on your list, with your certificate attached. It can even print and mail it to New York Life for you: no printer, no stamps. It also keeps track of who's done. The checklist is free.
