American Family Insurance's steps
Update personal information, including a new last name, in My Account or the MyAmFam app, or ask your agent. Life insurance uses a separate name change form.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
Let Sasha handle the letters
Sasha writes and signs your name change letter for American Family Insurance and every other company on your list, with your certificate attached. It also keeps track of who's done. The checklist is free.
