How to change your name with Lincoln Financial (life insurance)

Lincoln Financial's steps
Fill in both pages of the Life Name Change Form, tick marriage or divorce, attach a copy of your certificate or decree, sign, and email, fax or mail it. Annuities use the Contract Change Request.
Have these ready
- A copy of your marriage certificate, divorce decree or court order (most companies ask for one).
- Your updated driver's license or ID, if you have it. Social Security comes first, then the DMV.
- Your account number, and the name the account is in now.
